Quick answer

How do I organise life with a long-term condition?

Keep one page that summarises your care plan, track only what your clinician asked for, prepare an agenda for every appointment, record medicines exactly as labelled with refill dates, pace your energy, agree a written flare plan with your team, and review every three months. The Long-Term Condition Manager kit from Bodhih Training gives you every tool for each step.

Sound familiar?

You already feel the problem. Here is what it is costing you.

Appointments end before your questions do

You wait weeks, get fifteen minutes, and remember the important question in the car park. Without an agenda and your numbers to hand, the time slips away.

Paperwork in a drawer, not a plan

Letters, results and prescriptions pile up, but nobody has given you one page that says what the plan actually is. A new professional asks, and you cannot say.

Refills run out on a Saturday

Several medicines from several prescribers, different brand names on the boxes, and no simple way to see what runs out when.

Boom, bust and bad surprises

Good days become catch-up days, and the bill arrives two days later. Flares mean making decisions from scratch, in pain, at 2am.

Work, money and family feel like extra jobs

What do you tell your manager? What can you claim? Why does your family keep checking on you? Each question takes energy you do not have.

Now imagine this

Picture the next few weeks with the kit open beside you.

Imagine a Sunday evening a few weeks from now. You open the workbook, glance at the Dashboard, and see that one item says Order now. You order it. You add two questions to the bank. Fifteen minutes, and the week's health admin is done.

Picture your next appointment. As you hand over your agenda, your first question is at the top and your numbers sit beneath it. You ask about benefits, risks, alternatives and what happens if you wait. Before you leave, you write down the next step, who owns it and by when.

Now think of the next bad day. Instead of deciding everything from scratch, you read the flare plan you completed with your clinician, send the message to work that is already written, and pick up the flare bag from the wardrobe. Your family knows the signal and what helps.

Whether you start with the Medicines Record tonight or the Appointment Agenda before your next visit, the system builds itself one small step at a time, and your clinical team stays in charge of the clinical decisions.

A taste of Chapter 5

Many long-term conditions involve limited energy, pain or fatigue that varies from day to day. On a good day it is natural to do everything you could not do on the bad days. The trouble is that the bill often arrives later, sometimes a day or two later, which makes it hard to connect cause and effect. Pacing means spreading activity so you stay within what your body can manage, on good days as well as bad. The details of pacing differ by condition, and some conditions have specific clinical guidance about activity and exercise. For example, NICE in the UK has a guideline on ME/CFS that discusses energy management. Agree your approach with your clinical team. What follows is a way to organise whatever they advise.

Questions buyers ask

The Long-Term Condition Manager: FAQs

Read the free guideAlready bought? Resend my link

Is this medical advice?

No. The kit is educational and organisational. It never tells you what to take, what your numbers should be, or when to change anything. Every clinical word in your plan comes from your own doctor, nurse or pharmacist. In an emergency, call your local emergency number.

Does it work for my condition?

It is condition-neutral. The workbook's Settings sheet holds the measures and ranges your own clinician gave you, so it fits diabetes, asthma, arthritis, heart and thyroid conditions, IBD, chronic pain, long COVID, MS and many others.

Can I use it to manage a parent's or partner's condition?

Yes. Every tool works for carers, and the book explains how to organise while the person stays in charge of their own decisions. There is a household agreement, a family guide and space for carer respite.

How do I prepare for a specialist appointment?

Pick your top three questions with the most important first, bring your recent numbers and medicines list, and ask the BRAN questions for any decision: benefits, risks, alternatives and what happens if you do nothing. The Appointment Agenda form walks you through each step.

What is the best way to track symptoms?

Track only what your clinician asked for, plus one thing that matters to you, at the same time each day. Look at weekly averages and repeated notes rather than single readings. The Daily Log calculates 7-day averages for you.

How do I tell my employer about a chronic illness?

Decide who needs to know and share the effects and what would help, not your full history. Propose a trial with a review date and confirm it in writing. Rules differ by country, so check your HR team, company policy and local law; the kit includes dated country notes.

Is my health data safe in the workbook?

The files stay on your own device or storage. The book explains how to password-protect them, who to share what with, and how to record consent in your household agreement.

How can I measure my starting point?

Three AssessAll assessments fit this kit: the Reading Statistics in News, Advertising and Social Media Assessment for Everyday Readers (for judging health claims), the Assertive Communication Profile for Individual Contributors and Emerging Leaders (for appointments and work conversations), and Resilience & Stress Management.

How do I turn the new routine into a lasting habit?

Attach each step to an existing cue with an if-then plan, such as logging the day after brushing your teeth. Jobulary's guide 'If-then plans that make habits stick' explains the method and helps you turn it into a development plan.

What is the refund policy?

Digital kits are non-refundable once downloaded. The exceptions are corrupted files that are not fixed within 3 working days, duplicate charges, or failed delivery.

How much does The Long-Term Condition Manager cost?

₹799 including 18% GST for buyers in India, or $24 for buyers outside India. One-time payment, no subscription, with an automatic invoice.

How do I get the files after paying?

Instantly. Your download page opens the moment payment succeeds, and a permanent download link is emailed to you. Download each file separately or everything in one ZIP.

What software do I need?

Any of the usual tools: Excel or Google Sheets for spreadsheets, Word or Google Docs for templates, Adobe Acrobat Reader, Preview, Xodo or GoodNotes for the fillable PDFs, and Google, Outlook or Apple Calendar for the calendar plan.

Can I get a refund?

Because it is a digital download, kits are not refundable once downloaded. We always refund a duplicate charge or a failed delivery, and if a file is corrupted and we cannot fix it within 3 working days. Full terms: bodhih.org/refunds.

Can my company buy this for a team?

Yes. Team and company licences, and workshops on this topic run by Bodhih Training, are available. Write to solutions@bodhih.com or call +91 99000 11601.
