How do I prepare for a hospital stay and plan a safe discharge?
Plan backwards from the admission date, ask about benefits, risks, alternatives and doing nothing, keep a dated log of who said what, and ask on day one what must be true before discharge. The Hospital Stay and Recovery Planner from Bodhih Training gives you the e-book, forms, tracker and scripts to do each step.

You already feel the problem. Here is what it is costing you.
Eleven conversations, three versions of the plan
Staff change by the shift and everyone is kind and busy. Without a record, you cannot tell who said what, and every visit starts with the same questions.
Discharge arrives as a surprise
A nurse says home this afternoon. The equipment has not arrived, nobody has the medicines list, and the lift home is at work forty minutes away.
Is this normal, or should we call?
On day three at home the patient is more tired than on the ward. You have a discharge leaflet somewhere and a search engine full of frightening answers.
One person carries everything
Updates for nineteen relatives, claims, appointments, meals and work. The main carer is running on empty and nobody has divided the load.
Picture the next few weeks with the kit open beside you.
Picture the evening the admission letter arrives. Instead of a knot in your stomach, you open the tracker, type the date into Settings and watch every task fall into place with a due date and a name next to it. You print the Pre-Admission Plan, slide it into a clear folder, and notice how much calmer the house feels already.
On the ward, as you write your first five-line entry in the Ward Log, something shifts. You know the name of the nurse in charge, the time the doctors do their round and the three questions you came to ask. When two messages do not match, you check politely, using the exact words from the script, and you get a clear answer.
Imagine discharge day. You asked about the six areas on day one, so the equipment is fitted, the pharmacist has gone through the medicines list with you and the Decision Card is filled in from the team's advice before you reach the car. The family gets one calm update at seven, and you get an early night.
Whether you start with the tracker tonight or the Questions form tomorrow, the kit meets you where you are. And six weeks later, when you file the folder for next time, you will know exactly where everything is.

Hospitals run on teams and shifts. The person who knows the most about the patient today may not be on duty tomorrow, and handovers between shifts, however careful, are a known risk point. Families cannot fix that, but they can do one thing that helps a great deal: keep a simple written record of who said what, and when. A log does three jobs. It helps you remember. It helps you spot when two messages do not match, so you can ask a polite clarifying question. And it gives you something specific to refer back to: "On Tuesday at 10 am the physiotherapist said the plan was stairs practice on Thursday. Is that still the plan?" lands very differently from "Someone told us he'd be home by now."

Hospital Stay and Recovery Planner: FAQs
Read the free guideAlready bought? Resend my linkIs this medical advice?
No. The kit is educational and organisational. It helps you prepare, ask questions, keep records and plan practical support. It does not tell you what is normal for a patient, what treatment to choose or how to take any medicine; the clinical team does that. In an emergency, call your local emergency number.
What is in the Hospital Stay and Recovery Planner?
An e-book with 10 chapters and dated country notes, a 14-sheet tracker workbook, five fillable PDF forms, six Word templates (42 messages, letters, a patient summary, a care agreement, a worked example and a 29-prompt AI pack), three cheat sheets, a start-here map, a calendar file and a task board import file.
How do I prepare for a planned hospital stay?
Work backwards from the admission date. Two weeks out, sort people and paperwork; one week out, home and work; two or three days out, pack and confirm the instructions with the hospital. The Pre-Admission sheet works out the due dates for you once you type the admission date.
What questions should I ask before a procedure?
A simple set is benefits, risks, alternatives and what happens if you do nothing for now, as on NHS Greater Glasgow and Clyde's It's OK to Ask page. The kit adds two practical ones: what recovery involves and what you need to arrange. Then say the answers back to check you understood.
What should we ask before someone leaves hospital?
Ask early what must be true before discharge, then check six areas: clinical readiness, a written medicines list, the discharge letter and advice on who to call, follow-up appointments, equipment and home, and people and transport. The Discharge Checklist form and sheet cover each one.
Can I use it after an emergency admission?
Yes. Chapter 3 and the emergency quick-start on the 00 Start Here map cover the first hour and the first 48 hours. From there, the ward, discharge and recovery tools apply as they do for a planned stay.
Does it work in my country and currency?
Yes. The tracker has a Settings sheet for your own dates, currency, thresholds and phone numbers. Rules on leave, cover and legal authority differ, so the e-book keeps country specifics in a dated appendix for seven countries and tells you which official source to check.
How should I keep the health information private?
Store the tracker and scans on a password-protected device or private account, share only what the patient agrees to, and remove names and ID numbers before using any AI assistant. The Family Care Agreement has a section to record the patient's wishes on sharing.
How can I measure my starting point as a carer?
If you want an objective view of the skills involved, AssessAll offers the Family and Elder Caregiving Judgment assessment, the Assertive Communication Profile and an Attention to Detail and Error Checking assessment. They are optional and separate from the kit.
How do I turn this into habits that stick, or a development plan?
Pick one habit, such as the daily recovery log or the five-line ward log, and write it as an if-then plan: if the ward round finishes, then I phone at 11. Jobulary has a short guide to if-then plans that helps you turn it into a simple development plan.
How much does Hospital Stay and Recovery Planner cost?
₹799 including 18% GST for buyers in India, or $24 for buyers outside India. One-time payment, no subscription, with an automatic invoice.
How do I get the files after paying?
Instantly. Your download page opens the moment payment succeeds, and a permanent download link is emailed to you. Download each file separately or everything in one ZIP.
What software do I need?
Any of the usual tools: Excel or Google Sheets for spreadsheets, Word or Google Docs for templates, Adobe Acrobat Reader, Preview, Xodo or GoodNotes for the fillable PDFs, and Google, Outlook or Apple Calendar for the calendar plan.
Can I get a refund?
Because it is a digital download, kits are not refundable once downloaded. We always refund a duplicate charge or a failed delivery, and if a file is corrupted and we cannot fix it within 3 working days. Full terms: bodhih.org/refunds.
Can my company buy this for a team?
Yes. Team and company licences, and workshops on this topic run by Bodhih Training, are available. Write to solutions@bodhih.com or call +91 99000 11601.
